CHF Story: Gregory

Gregory is an 82-year-old grandfather of three who was diagnosed with congestive heart failure a year ago. Recently, he experienced an exacerbation. He enjoys meeting friends for book club and spending time with his family. Over the last few weeks, it has become more challenging for him to attend book club due to swelling in his feet, making ambulating more tiring. When he pushes through to do normal activities, he finds himself short of breath and needs to take frequent breaks. He monitors his blood pressure but does not check any other vital signs. His wife, who lives with him, encouraged him to call his provider to discuss the changes in his condition.

After speaking with his cardiologist, Gregory admitted that he takes his “water pill” (diuretic) every other day instead of daily because the frequent need to use the restroom is inconvenient. His wife prepares meals without adding extra salt but does not monitor sodium levels on food labels. The provider recommended home health care for education on slowing the progression of CHF. Gregory and his support system are willing to participate in home education to help him return to outings with friends and enjoy quality time with his family.
