In-Home Diabetic Care for Seniors

Our Diabetes Home Care Services were designed to deliver personalized and specific physician directed orders to clients with both newly diagnosed and chronic diabetic care needs. Our specially trained nurses and therapists work directly with the client’s own physician to achieve optimal treatment goals focused on controlling blood sugars with diabetic medication and healthy diet modification, exercise routines and compliance. Our diabetes’ educator team will provide personalized education and lifestyle recommendation and training to achieve best outcomes. We then monitor and record each diabetic patient’s progress daily, weekly or monthly as needed ensuring quality of life and optimal results are achieved.

Managing diabetes with in-home care

Treatment for type 1 and type 2 diabetes are heavily reliant on proper health care routines, which is one of the ways a home health skilled nurse can help. Type 1 diabetes is treated by administering insulin, typically through an injection insulin pen or a syringe to regulate blood glucose. Type 2 diabetes must be monitored carefully but often does not require drawing blood as much as Type 1 diabetes. Insulin therapy may still be used as a treatment for Type 2 diabetes; however, in cases that are less severe, this form of diabetes can often be managed through a healthy diet and sufficient physical activity. In-home care nurses can help with home health diabetes management by planning meals and administering diabetes medication, when applicable, in addition to helping patients cope with all other aspects of diabetes.
